You aren't trapped here, meaning you can fast travel away to a previously unlocked location, but you are free to explore this high level end-game area if you choose. You know you're there when the message 'Leaving Yara, Turn Around!' appears on screen. However, from any time then onwards, you can enter a boat (or we imagine, a plane) and travel to the edge of the map to leave Yara, which will play the secret ending.
